The Lung Institute collects general identification, contact, and credit information. Credit card information is used for billing purposes only. We may record calls to or from patient coordinators and customer service representatives to ensure accuracy and quality assurance, and to assist with internal performance reviews and training purposes.
How We Use Information
We use collected data to aid in providing products and services to our patients, including customer service, accounting, billing, collections, and for marketing purposes.The Lung Institute may provide anonymous, aggregate information to third parties.
The Lung Institute does not share personal information with third parties except as disclosed in this policy. The Lung Institute may provide personal information to subcontractors and professional advisers, all of whom shall be obliged to protect information privacy.
Personal information is stored in both paper and electronic files. All data is protected in a manner commensurate with its level of confidentiality and sensitivity.
Patients may review their own personal information contained in the Lung Institute’s files by contacting the Lung Institute’s privacy officer. Patients may correct erroneous information by contacting the Lung Institute’s privacy officer.
The Lung Institute may amend this policy from time to time. If such an amendment affects how the Lung Institute uses personal information, the Lung Institute will request consent before implementing the amendment.
Notwithstanding the general terms of this policy, the collection, use, and disclosure of personal information may be made outside of the terms herein to the extent provided for in any applicable privacy or other legislation in effect from time to time.
